Evolve Psychiatric Services does not bill insurances but does provide patients with the documentation needed to apply for reimbursement from out-of-network insurance benefits. Please contact your insurance company directly for an explanation of your benefits and the eligibility for out of network coverage.
Please make every effort to arrive on time. Appointments must end at the scheduled times regardless of when the patient arrives for the appointment. Patients are often scheduled back-to-back; therefore, we are unable to extend appointment times.
Cancellation policy: Please cancel appointments by telephone at least 48 hours prior to the appointment. Failure to cancel appointments will result in a charge at the regular rate for the canceled/missed session.
